Eminem finding the Slim Shady persona (he thought of Shady on the toilet didnt he? Close enough) after all that random spitting:

Coming back up from the "underground" i.e. from his hiatus. Simple double meaning. Using the title in two ways:
Glad bags, yeah glad to be back, cause last year was a tragedy
That landed me smack dead in rehab
Fucking doctor, I didn't understand a damn that he said
I planned to relapse the second I walked out of that bitch
Two weeks in brighton, I ain't enlightened
Summing up the crazy random theme of the album:
That means is just a regular day/ and this is the kind of shit I think of a regular lay
Pretty self explanatory:
Tell the critics I'm back and coming/to spit it back in abundance
Then the rest of the verse is more random spitting, attacking horror characters, that he starts in the previous verse and is the theme that he uses throughout the song. Didn't he get addicted to watching horror films when he got clean? Makese sense.
Underground is the PERFECT song to end Relapse as it sums up the album in many ways: using seemingly random violence/serial killing as a veil for his drug problems. As hiphopdane says Em said before that he uses the last track to sum up the album and this is no exception.
