Back when he had balls. It wasn't a matter of "oh damn, I can't say his/her name legally on a song." It was "well, they say I can't say his/her name but that's bullshit so I will. Sue me."
I think he's not saying her name out of respect for her as much as the legal aspect. He was pissed at his mom's lawyer, and decided fuck it, I'm gonna say it. I sincerely doubt he's still pissed at Kim; moreover, I'm pretty sure in Going Through Changes he said he'd always love her. In this case I don't think it's because he doesn't have "balls".
