The title 'My Darling' is an anagram for 'Damn Girly', as it's Em's way of saying 'damn girl, you so fine'. So clearly it's a song to all his female tween fans who bump his music whilst fisting themselves to Bieber pictures. It was going to be released as the first single to Recovery, but Interscope realized people might not understood the deep metaphorical meaning, so released Not Afraid instead.

At face value it's about the demon he sold his soul to to become famous returning to him after he 'killed' it in WIG. This demon is Slim Shady.

Could be about the illuminati, drugs, etc.

He refers to Slim Shady as "My Darling" because without Slim Shady he wouldn't be nearly as big as he is. He's trying to find the line between loving the character and hating the lifestyle behind the character.
