Perhaps I'm missing the concept and I think it's a good video I just am having trouble understanding the meaning.

Only, the second Eminem is basically the one who keeps wanting to change everything, but it keeps ending up that way because in the end, no matter what, Em kills her then kills himself. The second Eminem is basically a spectator, him looking back I guess and wanting to change shit but can't. That's why the second Eminem is always the one who is rapping.
At least, that's what I think.

The second Em is his multiple personality telling him not to trust this bitch because she's going to kill him (hence the evil look in the back of the car.) When they get to the hotel room he figures he has to kill her before she kills him but it turns out she was just a figment of his imagination. He realizes she's not there and that he's lost his mind so he kills himself. I assume when he was walking down the road in the beginning he probably just escaped a mental hospital. That's my theory

How could she have cheated on him when they just met? In the beginning of the video she picks him up on the road to give him a ride. They're talking like they just met. There's nothing personal about the way they interact. Also the second Em who's observing everything looks really angry when he gets in the back of the car not like someone who is remorseful and wants to change things.

I originally thought what you said too, but I don't think it makes a whole lot of sense that way, because then why would he follow her after he saw the text?
