I think that this what happened, based on all of the accounts, including the ones that were released when the name change came.
When he originally was recoridng for Relapse, he said he had recorded about 100 tracks, which to mean doesn't mean they were all finished (writing, recording, mastering, etc.). I think these could have included older tracks like the ones he did with TI when they recorded together, and also Insult to Injury and Keys to the City.
From those 100 tracks, he had enough to release two albums (obviously assuming that the tracks done were picked for 2 albums, though we know that may not be true as, for instance, 3am replaced what obviously should have been My Darling as the first song on Relapse-which may indicate they simply switched the songs OR that he was still recording and did 3am after the original "100 tracks").
After Relapse dropped, he was planning on releasing R2 with the material he had already recorded, but as time passed he started recording new material, and the new material he thought was better and/or decided if they were going to be on R2 then it didn't fit with the other songs already done. From there, he decided he was taking the album in different direction, started working with different producers and artists, and then decided that what was going to be on R2 was not going to anymore, which is why we got Refill.
After Refill dropped, and he had recorded enough for the album, he decided that it no longer fit the concept of Relapse, and therefore changed the name to Recovery.
I think that's a good timeline and explanation, anyone feel free to add/take away from I wrote, but I feel that's pretty much the general idea of what happened.
