No...you can completely switch up flows, it has nothing to do with your multis.
Just look at all of Em's songs, he is the master of switching up his flows.

It has ALOT to do with your multis. Where you put your in/end-rhymes is essentially your rhythm. Em isn't the same as Diabolic because Em knows when to drop & switch around the rhyme scheme to switch his flow, but Diabolic goes for using the same multis in same spots (Uhh, everywhere) in every sentence, so yes, he tends to sound the same.
