To anyone who thinks this song is about drugs, I hope you're not a native English speaker because if you are, that's really sad. The song is definitely about a girl, the question is whether it's about an actual girl or not. From listening to the song multiple times, if anything, this song is like another Stan. Each verse is another stage in the relationship between this man and a woman. In the first verse, the guy has just fallen in love with the girl, but he doesn't trust her completely. He doesn't open up to her and he doesn't know if he can trust her, even though he wants to. In the second verse, they're together and the relationship is amazing. Even then though, the guy isn't completely open with the girl and he isn't himself around her. Even though he loves her he hasn't opened up to her yet. In the last lines of the 2nd verse he says "Promise me if I cave in and break and leave myself open that I won't be making a mistake". In the third verse, it's obvious that he's opened up to the girl but she doesn't want him anymore. He kills her, then shoots himself.

When people are WITHOUT the drugs they're dependent on they get the shakes, not when they have them. It's about being nervous/feeling so much for this girl, nothing more.

Eh, the feeling of caving in and surrendering himself to this girl. After he has been hurt so many times already in the past ("coz ive been treated like dirt before ya"). Just because he mentions the words "cave in" it doesnt mean he is talking about drugs, lol.
