It's obviously about drugs, since the year & 6 months (or November 31st/May thing in STIW) was in Space Bound, and Space Bound was clearly about drugs, as the space/drugs metaphor is as old as time.

Still, the 18 month thing doesn't really make sense. He and pills had a 4-5 year relationship.
