Re-Up Booklet Question

As I havent gotten it yet, could someone tell me what exactly the booklet entails? I know someone scanned some pages, but they were kind of blurry, and I'm not sure if it was all of them.
If someone would like to put some better quality pictures up that would be awesome!!!
Also, what kind of text is there? Does it have lyrics, any kind of dedications, etc?
