That could be a contributing factor.
Interesting thought, EmBase. Since he can't use prescription pain killers, he may still be experiencing pain, thus making his movements more inhibited.
Like you said, it would be logical to assume that, based on his condition, he may try to compensate by moving "in" the beat.
Or...perhaps he's uncomfortable with being on the stage. Maybe a combination of these factors.
